Youth Application (Summer Programs)

All five of the following forms must be completed & submitted.

If a student already has a swimming certification on file,
it is not required to retest.

Starting this year (2008) we require that all applications are submittied with a $25 deposit to be returned at the end of the season.

Also, all youth sailors may sign up for a maximum of two sessions.

Please print the following forms, fill them out, and either bring them in to the sailing center during business hours, or mail them to the sailing center at:
